| Application to Conduct DIARS Revenue Events/Activities: |

This form is used by University of Delaware coaches and Recreation Services to conduct revenue events and activities. Complete and submit this form online. The NCAA Compliance Office will route the form for approval signatures. |

| Agreement to Participate: |
|
This form is to be used only when conducting alumni contests, clinics, and outside group activities. Coaches and Recreation Staff print and distribute this form to participants. |

| Approval of Published Materials |
|
This form is used by University of Delaware coaches and Recreation Staff in conjunction with conducting a revenue event. This form is to be submitted to the NCAA Compliance Office with all written materials that will be externally circulated relative to the event. |
