Summer Basketball League Permission Form:

This form is used by University of Delaware coaches and student-athletes to request permission to participate in summer basketball leagues.

Voluntary Quit Form:

This form is used by University of Delaware coaches to confirm, in writing, that a student-athlete has voluntarily quit the team.

Official Visit Forms

Official visit paperwork must be sent, with the BER, to the Delaware Field House Business Office.

Official Visit Summary Form:

This form is used by University of Delaware coaches to report an Official Visit taken by a prospective student-athlete.

Official Visit Student Host Fee Form:

This form is used by University of Delaware coaches to complete required paperwork for an official visit. The student host needs to read and sign this form.

Official Visit Mileage Reimbursement:

This form is used by University of Delaware coaches to request reimbursement for mileage costs related to a prospective student-athlete taking an official visit.

DIARS Revenue Events/Activities Forms

Please review the Policies and Procedures for DIARS Revenue Events/Activities before completing these forms.

Application to Conduct DIARS Revenue Events/Activities:

This form is used by University of Delaware coaches and Recreation Services to conduct revenue events and activities. Complete and submit this form online. The NCAA Compliance Office will route the form for approval signatures.

Agreement to Participate:

This form is to be used only when conducting alumni contests, clinics, and outside group activities. Coaches and Recreation Staff print and distribute this form to participants.

Approval of Published Materials

This form is used by University of Delaware coaches and Recreation Staff in conjunction with conducting a revenue event. This form is to be submitted to the NCAA Compliance Office with all written materials that will be externally circulated relative to the event.

Online Forms

Complete the form online, and it will be submitted via e-mail to the NCAA Compliance Office.

Length of Season Report:

This form is used by University of Delaware coaches to designate their length of season.

Renewals for Athletically Related Financial Aid:

This form is used by University of Delaware coaches to report athletes who will be renewing their athletically related financial aid. Student-athletes not returning to UD, and not renewing their scholarships, need to be listed as well.

Team/Individual Countable Athletically Related Activity:

This form is used by University of Delaware coaches to report practice activities. Coaches are to complete this form on a bi-weekly basis. Print it out and obtain the Team Captain's signature before submitting it to Coach Ferguson.

Awards Summary:

This form is used by University of Delaware coaches to record awards given during the past academic year. Do not record letter winner awards and/or MVP awards distributed at the year-end Men's and Women's Banquets.

Preliminary Roster:

This form is used by University of Delaware coaches to report their preliminary roster.

Roster:

This form is used by University of Delaware coaches to report their final roster.

Master Contact/Eval form:

This form is used by University of Delaware coaches as a recruiting tool to document contacts and evaluations of prospective student-athletes.

Recruiting Log:

This form is used by University of Delaware coaches as a recruiting documentation tool.

Telephone Logs:

This form is used by University of Delaware coaches to record phone call contacts with prospective student-athletes.

Institution Request List:

To be used by University of Delaware coaches to list incoming freshmen with the clearinghouse.
