After receiving the approved application with all approval signatures, this form is to be submitted by the requestor, with all written materials that will be externally circulated relative to the event.

Submit the form, with published materials attached, to the Assistant Director, Compliance & Operations. Upon approval, the Coordinator will send it to the Sports Information Associate for review and approval. It will then be sent to the Marking Associate for review and approval. The Marketing Associate will return the completed form to the Supervisor, Events at least two weeks prior to the scheduled event for distribution of copies and filing.
